We are accepting new patients for appointments! Office visits are by appointment only. We offer same day sick visit appointments, and same day or next day sport physicals.
Cancelled or Missed Appointments
As a courtesy to our physicians and other patients, please notify us if you have to cancel your appointment or are going to be late. If you are more then 15 minutes late for your appointment you may be asked to reschedule your appointment. Please note, if you do not notify our office 24 hours in advance of a cancellation, we reserve the right to assess a $25 fee or a $50 fee during the months of July, August and September.
Please call the office during regular office hours if you need a refill on a prescription. We recommend you call 2 to 3 days before you need the prescription refilled. Please remember that the medications commonly prescribed for ADD or ADHD can not be phoned in or sent electronically.
Our physicians are available on call 24 hours a day for emergencies. To contact the doctor on call, please call our answering service at 312-649-2576. If it is a life-threatening emergency, please call 911. If your child has injested a poison, please call poison control 1-800-222-1222.
We accept most major forms of health insurance including private insurance, PPO, HMO (through Advocate Health Partners) and Illinois Public Aid through BCBS Community Health and Meridan Health. At this time, our Public Aid panels are closed except to existing patients and newborns. We also accept Tricare. Please call our office with questions.
Co-pays are due at at time of service. We accept cash, check and credit card for payment. Please ask about our self pay pricing for physicals and vaccines.
We participate in the Vaccines for Children program (VFC) for patients with Illinois Public Aid or those without insurance.
The office observes the following major holidays and will be closed on these days: New Year's Day, Memorial Day, Fourth of July, Labor Day, Thanksgiving, Christmas Eve and Christmas Day. All other bank and school holidays we will remain open.
